Do I have to sign a care agreement
No. Care is a separate service and you can end it at any time. The site stays yours, along with the code and the logins.
Tying a client to a site they have no access to is common in this industry and we consider it dishonest. Access to the domain, the hosting and the code is yours from day one, whether or not you take the care plan.
Care makes sense when you do not want to remember about updates and want someone who picks up the phone when something stops working. If you have a person in the company who handles that, you can happily skip it.
The care agreement runs month to month, with no notice period counted in quarters. If the service stops being worth it, you end it at the close of the month.
